| Athletic Participation Form | This form is required for your student athlete to play football. It cannot be substituted with a doctor's form. Take this form to the doctor to complete prior to August each year. It is required for your student athlete to play football. It is only good for one year and must be completed each year. |
| Emergency Treatment Consent Card | This form must be completed for your student athlete to play. Please ensure all information is current and correct. |
| Player/Parent Contact Form | This form is used as communication between the Football Boosters and the Football players and families. Please ensure all information is current and correct. |
| Waiver for Media Publishing | This form is for the PHHS Football Boosters to authorize the use of players name, statistics and/or photo on the Booster website, flyers or any Booster communications. |
