Take this form to the doctor to complete prior to August each year. It is required for your student athlete to play football. It is only good for one year and must be completed each year. |
| Athletics Emergency Treatment Consent Card | This form must be completed for your student athlete to play. Please ensure all information is current and correct. |
| Player/Parent Contact Form | This form is used as communication between the Football Boosters and the Football players and families. Please ensure all information is current and correct. |
| Waiver for Media Publishing | This form is for the PHHS Football Boosters to authorize the use of players name, statistics and/or photo on the Booster website, flyers or any Booster communications. |
| CIF Ethics in Sports Form | The CIF - San Diego Section encourages and assists high school students, through participation in interscholastic athletics, to develop and strengthen values, attitudes, and skills that will promote their personal growth as individuals and as members of a diverse society. |
| Athletics Signature Card | This card only needs to be signed once each year. It is on green cardstock and cannot be downloaded on the website. It will be provided to your athlete for parent signature. |
